ECOA - This form is for the Equal Credit Opportunity Act (ECOA). THe ECOA is a federal law that requires lenders and other creditors to make credit equally available without discrimination based on race, color, religion, national origin, age, sex, marital status or receipt of income from public assistance programs.

Certification and Authorization - This form is to confirm that the information in a loan application is correct and complete. It also gives the authority to release the information to the necessary parties.

Lock Agreement - This form is for a mortgage rate lock-in. A lock-in is a lender's promise to hold a certain interest rate and a certain number of points for you during the period of time specified by the lender while your loan application is processed.

Origination Agreement - This form is to guarantee the mortgage broker as the sole contractor for a mortgage engagement.
